(If
you would like to contribute an amount other than those listed you may
use the 'Quanity' feature as a multiplier to create most any amount.
e.g. a $35 monthly donation could be made by selecting the $5
donation from the list and selecting "7" in the 'Quantity' box: 5 x 7=$35)

This initial contribution will begin your monthly tax deductible donations. In subsequent
months, around the 15th of each month, your credit card will automatically be charged
the same amount as this donation. If at any time you wish to change or
cancel your donation just contact me at the email address or number
below and I will comply with your instructions promptly.

You will receive an email confirmation receipt with each monthly
donation. Also, in January of each year a statement summarizing your
annual giving to the Cornerstone Forum for the prior year will be
mailed to you which you may use for preparing your income tax
deductions.
